Formidable Forms is a a premium plugin in the WordPress community. It has a helpful drag and drop interface where you can build and configure a form in minutes. Includes advanced features like conditional fields and many advanced integrations.

There is no set up needed once you have the plugin installed, activated, and enabled in the Client Report Settings.
